Iron is an important nutrient, essential in the production of healthy red blood cells (RBCs). It is a crucial part of heamoglobin, substance that helps RBCs carry oxygen and deliver it to the tissues. If low levels of heamoglobin are found via a CBC (Complete blood count) test, transferrin testing may be necessitated. The human body cannot make iron and thus, it is absorbed through the food and supplements consumed. Iron is transported throughout the body by a protein, transferrin. The rest is stored in the form of ferritin. Hence, an Iron test may also be recommended by your doctor to check iron levels.

The transferrin test directly evaluates the level of transferrin in the blood and the levels are dependent on liver function and the dietary intake. When the iron levels in the body decrease, liver produces more transferrin, indicating that the body needs more iron. When the serum iron levels decrease, smaller and fewer red blood cells are produced, leading to iron deficiency anemia.

Transferrin levels increase in iron deficiency. Pregnancy and oral contraceptives also elevate transferrin levels. Subsequently, transferrin levels decrease when there is an iron overload. Alcoholism and chronic liver disease also lead to an iron overload. Low levels of transferrin could also suggest poor production of transferrin by liver, an infection, or excessive loss of transferrin in the urine.

Transferrin test is often ordered when the doctor suspects the signs and symptoms of iron deficiency anemia such as fatigue and weakness, pale skin, headaches and dizziness etc, and also when there is iron overload. The signs and symptoms of iron load worsen with time. Joint pain, weakness, liver damage, fatigue and constant tiredness, are some of the symptoms of iron overload.

The test is a simple blood test where the blood is collected through venipuncture. There are certain risks associated with the procedure like infection, bruising, or pain at the site of the puncture but it fades away on its own. Precautions like letting your doctor know about all type of medications you take, staying hydrated, fasting prior to the test etc. should be taken. Do not take iron supplements prior to the test.
